INFORMATION FOR CONTRACTORS AND THEIR REPRESENTATIVES REGARDING THE PROCESSING OF PERSONAL DATA

This information is addressed to representatives of contractors and entities maintaining a business relationship with Przedsiębiorstwo Techniki Medycznej “Anes-Med” spółka z ograniczoną odpowiedzialnością sp.k., as well as persons using the ANESMED Customer Service Centre helpline.

The administrator of your personal data is Przedsiębiorstwo Techniki Medycznej “Anes-Med” spółka z ograniczoną odpowiedzialnością sp.k. with its registered office in Warsaw, ul. Senatorska 2, 00-075 Warsaw (“Administrator”).

  1. Purposes and legal basis for the processing of personal data

The Administrator processes personal data for the following purposes:

a) implementation of legal obligations incumbent on the Administrator, related to its activities, resulting from legal provisions, including tax and reporting obligations, regarding the storage of specific documentation, counteracting abuse (art. 6 section 1 (c) of the GDPR).

b) implementation of the Administrator’s legitimate interests, including: marketing of the Company’s products and services, management of relations with contractors, performance of contracts, ongoing correspondence, monitoring and improving the quality of services provided by the Company, determining, pursuing or defending claims, counteracting abuse, reporting and reporting obligations of the Company (internal and external (art. 6 section 1 (f) of the GDPR) and providing information or answering questions in the case of people using the helpline (art. 6 section 1 (f) of the GDPR)

  1. Information on recipients of personal data

In connection with the processing of personal data, the Administrator may share personal data with the following recipients or categories of recipients:

a) public authorities and entities performing public tasks or acting on behalf of public authorities, to the extent and for the purposes resulting from legal provisions, e.g. tax office, police, courts;

b) entities related to the Administrator, including within the capital group to which the Administrator belongs, in the implementation of reporting and reporting obligations, as well as in the case of the implementation of other legally justified internal administrative purposes of this group;

c) entities processing personal data on behalf of and at the request of the Administrator, including providers of IT, archiving, courier, legal, debt collection services.

  1. Information on the intention to transfer personal data to a third country

The Company does not transfer personal data to third countries, i.e. outside the European Economic Area.

  1. Period of personal data storage

Personal data of representatives of contractors and persons using the helpline will be stored by the Administrator for the period necessary to achieve the purposes of processing, but not longer than for a period of 3 years from the date of data collection.

  1. Data subject’s rights

In connection with the processing of personal data by the Administrator, the data subjects are entitled to the following rights:

a) the right to access personal data, including the right to obtain a copy of such data,

b) the right to request rectification (correction) of personal data if the data are inaccurate or incomplete;

c) the right to request the deletion of personal data (the so-called “right to be forgotten”) in the cases indicated in the GDPR;

d) the right to requests to limit the processing of personal data;

e) the right to object to the processing of data;

f) the right to transfer personal data;

g) the right to lodge a complaint with a supervisory authority – in the event that the Administrator considers that the processing of personal data violates the provisions of the GDPR.

Obligation to provide personal data

Providing personal data is voluntary, however, failure to provide them will prevent the Administrator from achieving the purposes of processing indicated in this information, in particular contact with the data subject, and thus with the Administrator’s contractor.
